Zone	Description	Management

Zone I—midline retroperitoneum	Extends from the aortic hiatus to the sacral promontory and is divided into supramesocolic and inframesocolic zones	All injuries require surgical exploration

Zone II—lateral retroperitoneum	Extends on either side from the renal hila to the pericolic gutters Avoid exploration in blunt trauma	Explore all penetrating trauma

Zone III—pelvic retroperitoneum	Sacral promontory and encompasses the pelvis Explore all penetrating trauma	Explore for expanding hematoma in penetrating trauma
