Pancreatic Beta-Cell Function during Oral Glucose Tolerance or Meal Test

Pancreatic beta-cell function includes several aspects that can be partially independent. For several years, it was believed that the first metabolic defect in people showing progressive deterioration in the glucose tolerance was insulin resistance. However, several subsequent studies showed that the impairment in beta-cell function appeared early and showed a role equally important to that of insulin resistance. Historically, the hyperglycaemic clamp and the graded glucose infusion test have provided fundamental information on beta-cell function. However, recent studies including large numbers of subjects—such as genetic studies, follow-up protocols, and drug trials—have called for methods of wider applicability, that is, based on simpler tests. Therefore, it has become increasingly important to assess beta-cell function during the oral glucose tolerance test (OGTT) and the meal test. The first aim of this special issue is to present the different methods for the assessment of beta-cell function during an OGTT or a meal test. The second aim is to include studies where such methods have been applied for the assessment of beta-cell function in different categories of subjects. The third aim is the study of the effects on beta-cell function of pharmacological agents used in the treatment of diabetes. Studies exploring the relationships of beta-cell function with different aspects of metabolism (such as lipid metabolism) will also be considered.
