Patient currently taking steroids		Moderate surgery	Usual perioperative steroid + 25 hydrocortisone at induction + 100 mg/day for 24 hrs
		Major surgery	Usual perioperative steroid + 25 hydrocortisone at induction + 100 mg/day for 48–72 hrs
	High dose immunosuppressive	Give usual immunosuppressive dose during perioperative period
Patient stopped taking steroid	<3 months	Treat as if on steroids
	>3 months	No perioperative steroid necessary
