Our antimicrobial prophylaxis for allogeneic BMT is as follows.

From day −3 to day +75 or till patient is immunocompromised due to likely GVHD management as fungal prophylaxis or amphotericin B 0.5 to 1 mg/kg once daily or on alternate days after ANC becomes less than 200; antifungal prophylaxis stops after neutrophil engraftment
Tab. of valacyclovir
1000 mg once daily
From day −3 to day +30 or longer in case of GVHD as Varicella zoster virus and Herpes simplex virus prophylaxis
Trimethoprim/sulfamethoxazole
10 mg/kg/day
Double strength daily from day −8 till day −2; it is restarted twice weekly as soon as engraftment is achieved
Tab. of penicillin V
250 mg twice weekly
From day +28 for prophylaxis against encapsulated organisms or tab. of amoxicillin-clavulanate for Pneumocystis carinii prophylaxis
Febrile neutropenia episodes were managed as per standard protocols.
