NFOG = new freezing of gait questionnaire; MoCA = Montreal cognitive assessment; LEDD = levodopa equivalent daily dose; ANART = American national adult reading test. LEDD = L-dopa dose + (L-dopa dose × 1/3) if on entacapone + (bromocriptine (mg) × 10) + (cabergoline or pramipexole (mg) × 67) + (ropinirole (mg) × 20) + (pergolide (mg) × 100) + (apomorphine (mg) × 8).
