Topical steroids; antihistamines
Widespread maculopapular rash
Hold lenalidomide; topical or oral steroids depending on severity; antihistamines; after resolution, restart lenalidomide at lower dose
Generalize erythroderma or desquamation
Hold lenalidomide; oral steroids; Dermatology consultation; do not restart lenalidomide
Urticaria
Hold lenalidomide; symptomatic management with antihistamines ± oral steroids; after resolution, may attempt desensitization if reinitiation of lenalidomide is planned
