Dose 1 = 2 mg/kg loading + 50 mg/kg/min (lidocaine) and 3 mg/kg loading + 10 mg/kg/min (ketamine). Dose 2 = 100 mg/kg/min (lidocaine) and 50 mg/kg/min (ketamine).
