<i>Background</i>. Elderly patients are often treated differently than younger patients, even when suffering from the same disorder. <i>Objective</i>. The study examines the influence of “patient age” on the perception of symptoms and conclusions of physicians in respect to diagnosis and treatment. <i>Methods</i>. In a randomised controlled experimental study on medical decision-making, 121 general practitioners were given two case vignettes which contained all the criteria for major depression according to ICD-10, but differed in respect to the age of the patient (39 or 81). Reaction time, diagnostic conclusions and therapeutic recommendations were assessed by computer. <i>Results</i>. Depression and anxiety were significantly seen as more probable in the young cases and dementia and physical illness in the old. In young age, psychotherapy, pharmacotherapy and referral to a specialist or inpatient treatment were significantly more recommended than in old age, for whom supportive counselling was significantly more recommended. The time needed for a decision was significantly longer in the older patients. <i>Conclusion</i>. Ageing stereotypes can also form medical illness concepts and have a significant influence on diagnostic and therapeutic decisions.

A Randomised Controlled Experimental Study on the Influence of Patient Age on Medical Decisions in Respect to the Diagnosis and Treatment of Depression in the Elderly

<i>Purpose</i>. To present the psychometric properties of a new measure of quality of life in older age, the Older People&#39;s Quality of Life (OPQOL) Questionnaire, compared with the CAPSE-19 and the WHOQOL-OLD. <i>Design and Methods</i>. The vehicle was three national population surveys of older people living at home in Britain, including a survey of ethnically diverse older people. <i>Results</i>. The OPQOL had acceptable levels of reliability and validity in British population samples of older people, but more modest in the ethnically diverse population sample. The CASP-19 and WHOQOL-OLD had acceptable levels of reliability and validity in the British population sample, but not in the ethnically diverse sample. <i>Implications</i>. The OPQOL has potential for use as a multidimensional population surveillance instrument for use with older populations, or as an outcome measure of multisector policy. Its strengths are that its development was embedded firmly in the perspectives of older people, integrated with theory.

The Psychometric Properties of the Older People&#39;s Quality of Life Questionnaire, Compared with the CASP-19 and the WHOQOL-OLD

Bronchial asthma is a common problem with enormous medical and economics impacts. It is an inflammatory disease of the airways associated with intermittent episodes of bronchospasm. Asthma is not uncommon in the elderly patients. Prevalence of asthma is similar in older and younger adults. Asthma in the elderly patient is underdiagnosed because of false perceptions by both patient and physician. The high incidence of comorbid conditions in the elderly patient makes the diagnosis and management more difficult. Correct diagnosis is demonstrated with spirometry. The goals of asthma treatment are to achieve and maintain control of symptoms and to prevent development of irreversible airflow limitation. Asthma drugs are preferably inhaled because this route minimizes systemic absorption and, thus, improves the ratio of the therapeutic benefit to the potential side-effects in elderly patients.

At baseline and after the intervention, subjects were submitted to a blood sample to determine their hematological profile (red blood cells, hemoglobin, hematocrit, platelets, leukocytes, neutrophils, lymphocytes, monocytes, mean corpuscular volume, mean corpuscular hemoglobin, mean corpuscular hemoglobin concentration, red cell distribution width). At baseline, no difference was observed between groups. Moreover, we found no significant difference after the intervention on any of these markers. A 6-month resistance program in healthy older individuals seems to have no beneficial nor deleterious effects on hematological blood parameters. However, resistance training was well tolerated and should be recommended for other health purposes. Further studies are needed to confirm these results in a large population.

Effect of Resistance Training on Hematological Blood Markers in Older Men and Women: A Pilot Study

Life history theory proposes that early-life cues induce highly integrated responses in traits associated with energy partitioning, maturation, reproduction, and aging such that the individual phenotype is adaptively more appropriate to the anticipated environment. Thus, maternal and/or neonatally derived nutritional or endocrine cues suggesting a threatening environment may favour early growth and reproduction over investment in tissue reserve and repair capacity. These may directly affect longevity, as well as prioritise insulin resistance and capacity for fat storage, thereby increasing susceptibility to metabolic dysfunction and obesity. These shifts in developmental trajectory are associated with long-term expression changes in specific genes, some of which may be underpinned by epigenetic processes. This normative process of developmental plasticity may prove to be maladaptive in human environments in transition towards low extrinsic mortality and energy-dense nutrition, leading to the development of an inappropriate phenotype with decreased potential for longevity and/or increased susceptibility to metabolic disease.

Impaired Perinatal Growth and Longevity: A Life History Perspective

<i  >Objective</i>. The purpose of this study was to investigate prognostic impact of cholesterol and its subfractions among 75-year-old people from the general population. <i  >Methods and Results</i>. The study comprised a random sample (222 women and 210 men) from the general population (participation rate 70&#x25;). During 10-year follow-up, 19&#x25; of women and 35&#x25; of men experienced a major cardiovascular event (MCVE). The all-cause mortality was 29&#x25; for women and 47&#x25; for men. Total cholesterol and low-density lipoprotein cholesterol (LDL-C) were not significantly related to prognosis in either sex. <i  >Main Conclusions</i>. HDL-C was associated with dismal prognosis in men but not in women.
