Cardiac Failure as an Unusual Presentation in a Patient with History of Amyotrophic Lateral Sclerosis

<table>Transthoracic echocardiography revealed global hypokinesia of the LV with left ventricular ejection fraction (EF) of 20% which was measured by biplane Simpson method. The three-year-earlier echocardiography had showed ejection fraction of 50%.</table>
