Evolution of SILS Cholecystectomy in the Caribbean: The Direct Transfascial Puncture Technique Using Conventional Instruments without Working Ports

<div>10 mm visual port is placed in the umbilical incision and a 5 mm Maryland’s grasper passes beside the visual port, both encircled by a purse string suture to create a seal. A 5 mm working instrument (cautery hook) is passed directly across the fascia outside the purse string suture.</div>
