As part of the full length research paper the supplementary material encompasses, location Map of Arba Minch district including all localities, full length English and Amharic (Official language used in Ethiopia) version questionnaire for both qualitative and quantitative studies.

The questionnaire has eight variable categories including Respondents’ Identification, Socio-Demographic Characteristics, Birth History, Knowledge of women about Birth spacing, Breast feeding practice , Knowledge and Practice of Modern Contraceptive use, Attitude of respondents towards birth spacing, fertility and family planning, and Household wealth status of respondents.
