Stapedius tendon was transected bluntly with a Wullstein needle. This resulted in unintended incudostapedial joint separation A prominent anterior canal bulge made the scope insertion and endoscopic guided insertion of instruments slightly more challenging but procedure was still completed using a 2.7 mm diameter scope
