<i>Purpose</i>. To analyze the current physician clinical oncological workforce within the CARICOM full member states with an aim to make recommendations for building capacity.<i> Methods. </i>A questionnaire was prepared and emailed to professionals working in oncology in 14 CARICOM full member countries. It was designed to identify the number of specialists providing hematology, medical oncology, and radiotherapy services.<i> Results</i>. Ten countries (71.4&#x25;) supplied information. Oncology services were insufficient in the majority of countries. Hematology proved to be the most adequately staffed with six countries (60&#x25;) having the recommended number of specialists. Medical oncology services were deficient in five countries (50&#x25;). Radiation oncology services were the most limited with nine countries (90&#x25;) unable to provide the required quota of specialists. The majority of the workforce consisted of nonnationals (55&#x25;). The remaining practitioners were nationals, and of these 50&#x25; were regionally trained. Oncological care was primarily offered within the public sector.<i> Conclusion</i>. Oncological staffing within the CARICOM full member states is insufficient to meet the demands of the current population. Encouraging training through locoregional or international programs is key to obtaining the numbers required. Cancer registries will help provide data to influence public policy and improve the oncological healthcare system.

The Caribbean Community Clinical Oncology Workforce: Analyzing Where We Are Today and Projecting for Tomorrow

<i>Objectives</i>. Locoregional recurrence is the predominant pattern of treatment failure in advanced head and neck cancers. Reirradiation is a useful modality to treat inoperable head and neck cancer patients with recurrent disease. The aim of the present study was to analyze the treatment toxicity and early clinical outcomes in patients undergoing reirradiation.<i> Methods</i>. Twenty patients of head and neck cancers with recurrences or second cancers were evaluated. Reirradiation was done using simultaneous integrated boost volumetric modulated arc therapy (SIB VMAT), intensity modulated radiotherapy (IMRT), or conventional radiotherapy using 6MV photons. Dose prescription ranged from 30 to 60 Gy in conventional fractionation.<i> Results</i>. Seventeen males and three females were evaluated in this analysis. The median age of patients under study was 56.5 years. At time of analysis 8 patients (40%) had a complete response, 7 patients (35%) had progressive disease, and 25% had partial response or stable disease. Grade III-IV mucositis, dermatitis, xerostomia, dysphagia, and trismus were seen in 20%, 20%, 50%, 35%, and 45% patients, respectively, during retreatment. Evaluation of Reirradiation in Locally Advanced Head and Neck Cancers: Toxicity and Early Clinical Outcomes

The aim is to summarize current knowledge on both QoL and depressive/anxious symptoms in patients with UM, including studies on the effect on QoL and psychological status of genetic testing related to the risk of metastatic disease. A review from the last 25 years by using the databases &#x201c;PsycInfo,&#x201d; &#x201c;Medline,&#x201d; and &#x201c;Science Direct&#x201d; was performed. As a total result, eighteen papers were retrieved. Eight studies (44.4&#x25;) used a prospective design methodology: two were retrospective observations (11.1&#x25;), three were cross-sectional observational studies (16.6&#x25;), and three (16.6&#x25;) were naturalistic follow-up studies. One trial was conducted with a case-control design (5.5&#x25;), and one was a methodological paper (5.5&#x25;). The number of subjects included in the studies ranged widely, between 7 and 842 (mean: 152.1 &#xb1; 201.3), for a total of 2587 patients, 1306 males (50.5&#x25;) and 1281 females (49.5&#x25;). The mean age of subject enrolled was 61.3 &#xb1; 4.1 years. Twenty-six different scales, questionnaires, or interviews were utilized. No significant differences in QoL between radiotherapy and enucleation emerged. Genetic testing did not significantly affect QoL or psychological status.

Quality of Life, Depression, and Anxiety in Patients with Uveal Melanoma: A Review

Some of the most significant advances in ovarian cancer treatment have been those that result in improvements in progression-free survival (PFS); however there is little research to understand the value that patients place on accessing therapies that result in PFS as a clinical outcome related to survivorship. This study therefore aimed to understand the experience and expectations of women with ovarian cancer in Australia in relation to quality of life (QoL) and treatment options. An online survey collected demographic information and 13 investigator-derived structured interview questions were developed to understand the experience of women with ovarian cancer, their understanding of terminology associated with their condition, and expectations of future treatment. This study demonstrated that ovarian cancer patients equate PFS with being in remission and that patients expect QoL during that time to be good to excellent. Women in this study described excellent QoL as feeling positive and happy and not worrying about cancer, feeling fit and healthy without side effects, and being able to live life as they did before their diagnosis, including the absence of fear of progression or recurrence. It is therefore suggested that there is a positive relationship between PFS and QoL. While it is difficult to quantify QoL and further research is needed, the results of this study suggest that the minimum time that women with ovarian cancer expect in relation to treatments that result in PFS is approximately six months. In the absence of this information, decision-makers are left to make assumptions about the value women place on access to therapeutics that increase PFS, which for this type of cancer is an important aspect of survivorship.

Experience and Expectations of Ovarian Cancer Patients in Australia

The aim of this study was to investigate the characteristics of individuals with intraoral potentially malignant disorders (IOPMD) in an oral pathology service in Brazil. Cases were screened based on clinical diagnosis of leukoplakia (LKP), erythroleukoplakia (ELKP), and erythroplakia (EP). Clinical data and information regarding associated factors were gathered from biopsy reports. Histological diagnoses were collected from histopathological records. Among 208 IOPMD cases, 84.13% involved LKP; 11.1%, ELKP; and 4.8%, EP. The most affected sites were the gingiva and buccal mucosa. Histologically, epithelial dysplasia was present in 66.8% of the lesions, acanthosis and hyperkeratosis without epithelial dysplasia were present in 27.9%, and squamous cell carcinoma was present in 2.9%. Most patients were males, fair-skinned, with mean age of 53.4 years. Chronic smokers represented 73% of subjects, of which 30% also consumed alcohol. Reanalysis of RNA-seq dataset from The Cancer Genome Atlas shows higher expression of IGSF9 in endometrial cancer versus normal control and expression was associated with poor prognosis. These results suggest IGSF9 as a new biomarker in endometrial cancer and warrant further studies on its function, mechanism of action, and potential clinical utility.

RNA-seq Reveals the Overexpression of IGSF9 in Endometrial Cancer

<i>Background</i>. There is little data regarding the effectiveness of palliative radiation with conventional fractionation for metastatic renal cell carcinoma (RCC), which has been described as radioresistant. We conducted a retrospective analysis of patients with metastatic bony disease from RCC treated with radiation therapy at our institution.<i> Methods</i>. Forty patients with histologically confirmed RCC with a total of 53 treatment courses were included. Pain response after radiotherapy was recorded and freedom from progression was generated using posttreatment radiographs. Patient data was analyzed to assess influence on local control.<i> Results</i>. Patients had a median age of 63. Median follow-up was 9.3 months. The most common radiation dose was 30 Gy in 10 fractions. Pain control after radiotherapy was achieved in 73.6% of patients. Radiographic control at 1 year was 62%.<i> Conclusions</i>. Pain response and control rates for osseous metastatic disease in RCC are comparable to other histologies when treated with conventional fractionation. These appear to be inferior to reported control rates from stereotactic treatments.
