Obtaining access into the peritoneal cavity by direct cutdown inside the umbilical ring.
(a) The incision is started. It will be 1.2 cm in length
(b) The umbilicus is everted as the incision is carried to the fascia
(c) A small hemostat is placed into the peritoneal cavity and the access into the peritoneal cavity is dilated to accommodate a multi-trocar port. As needed, the access is enlarged by incising the fascia
