<i>Background</i>. Inflammatory bowel disease (IBD) might increase the risk of rosacea; however, the relationship between IBD and rosacea is not determined. In this study, Mendelian randomization (MR) design was used to explore the potential causal association between IBD and rosacea. <i>Methods</i>. A two-sample MR was explored from large-scale genetic summary data from genome-wide association study (GWAS), including IBD (<i>n</i> = 59959) and its main subtype Crohn’s disease (CD) (<i>n</i> = 40266) and ulcerative colitis (UC) (<i>n</i> = 45975). Summarized data of rosacea (<i>n</i> = 1877) were obtained from different GWAS studies. Based on previous observational studies, our main analyses were conducted by the inverse-variance weighted (IVW) method with the random-effects model, with a complementary with the other two analyses: weighted median method and MR-Egger approach. <i>Results</i>. Our study provided potential evidence between genetically predicted IBD and rosacea, and the degree of association of different subtypes of IBD for rosacea is different. In addition, we found that the mechanism of IBD affecting the pathogenesis of rosacea is closely related to the variation of IL23R. Blocking the IL-23 signaling pathway might be a reasonable strategy to treat IBD and prevent rosacea. Understanding the specific relationship between IBD and rosacea provides the possibility to treat both clinically and guide the development of new drugs.

Inflammatory Bowel Disease and Rosacea: A Two-Sample Mendelian Randomization Study

<i>Background</i>. Crisaborole has been considered a promising alternative for topical treatment of atopic dermatitis (AD), mainly supported by AD-301 and AD-302. However, critical insights into these two studies have previously been proposed. <i>Objective</i>. To make a comprehensive assessment of the application of crisaborole in mild to moderate AD. <i>Methods</i>. A systematic review and meta-analysis were conducted, in which only randomized controlled trials comparing the application of crisaborole twice daily to vehicle or other active treatment in patients with mild to moderate AD were included. The selection of outcomes was based on the recommendation of the HOME initiative. Patient-reported symptoms, clinician-reported signs, health-related quality of life, and the safety of crisaborole were all assessed using appropriate measurement instruments. <i>Results</i>. Eight RCTs with 2266 patients were included in the pooled analysis. Compared to those treated with vehicle, patients on crisaborole experienced a greater improvement in NRS (MD −0.70; 95% CI −0.94 to −0.47), POEM (MD −3.50; 95% CI −4.34 to −2.66), EASI (MD −14.49%; 95% CI −18.24% to −10.73%), ISGA (RR 1.45; 95% CI 1.28 to 1.63), DLQI (MD −1.54; 95% CI −2.17 to −0.92), and DFI (MD −1.16; 95% CI −1.72 to −0.59) during the 4-week treatment. More patients achieved EASI 75 (RR 1.71; 95% CI 1.43 to 2.04) with crisaborole administration. There was no significant difference between two interventions in the incidence of AEs (RR 1.12; 95% CI 0.98 to 1.29), SAEs (RR 1.89; 95% CI 0.47 to 7.60), or AE-related withdrawal (RR 0.87; 95% CI 0.47 to 1.60). One RCT also made comparison between crisaborole and pimecrolimus, suggesting that no significant difference was detected in the improvement of EASI or NRS at most time points. <i>Conclusion</i>. High-quality evidence was provided to demonstrate that the short-term application of crisaborole is safe and efficacious for the treatment of mild to moderate AD. The practical efficacy of crisaborole is similar to that of pimecrolimus.

Topical Administration of Crisaborole in Mild to Moderate Atopic Dermatitis: A Systematic Review and Meta-Analysis

Peutz–Jeghers syndrome (PJS) is a rare genetic disorder characterized by multiple gastrointestinal polyps and mucocutaneous pigmentation. Abnormal pigmentation typically develops in infancy or childhood. As PJS-related facial pigmentation can lead to psychological burden due to its effects on esthetics, treatment is required. Herein, we report on the efficacy and safety of treatment of lip mucosal pigmentation using a Q-switched 755 nm Alexandrite Laser in children with PJS, aged 2–12 years of age. A topical anesthetic was used prior to the application of laser therapy. A treatment efficacy of 100% was achieved with one to three treatments, with an excellent outcome achieved in five cases (62.5%) and a good outcome in three (37.5%). Recurrent pigmentation was observed in one case over the 6-month follow-up period. There were no adverse effects, such as scarring or hyperpigmentation or hypopigmentation. The treatment did cause pain, apprehension, and crying in some children, requiring special attention. Although our sample size is small, our findings do provide support of the high efficacy and safety of the Q-switched 755 nm Alexandrite Laser for the treatment of lip mucosal pigmentation in children with PJS. Further studies are required to confirm these findings.

Clinical Efficacy of 755 nm Laser Treatment of Lip Mucosal Pigmentation in Children with Peutz–Jeghers Syndrome

Mucocutaneous lesions are the most frequent symptoms of Behçet’s disease (BD). Recently, new therapies are being used to treat refractory cases, but the effect of these treatments on mucocutaneous manifestations has been scarcely reported. Our objective was to describe the mucocutaneous response to the different therapies used to treat BD in routine clinical practice. We retrospectively reviewed the clinical records of all patients diagnosed with BD seen at our institution between January 2010 and January 2022. Patients with BD without mucocutaneous manifestations were excluded. We included 109 patients diagnosed with BD: 51 males (46.8%) and 58 females (53.2%). The mean age at diagnosis was 31.58 years (standard deviation (SD) 12.110) and the mean time of disease evolution was 14.94 years (SD 11.094). Oral ulcers were the most frequent symptom present in 100% of patients, followed by genital ulcers (GU) in 76.1% of patients. Twenty-four patients (22%) had severe mucocutaneous symptoms (>12 lesions/year) before treatment. Mucocutaneous Response to New Therapeutic Strategies in Behçet’s Disease: A Retrospective Cohort Study

Intralesional 5-fluorouracil (5-FU) and triamcinolone acetonide (TAC) injection is effective for the treatment of hypertrophic scar. The side effects of current that recommended 45 mg/ml (high-dose) 5-FU have been reported. However, no previous study has investigated the efficacy and safety of low-dose (2.5 mg/ml) 5-FU with 4 mg/ml TAC or medium-dose (10 mg/ml) 5-FU with 4 mg/ml TAC for treatment of hypertrophic scar. Herein, a retrospective comparative study was conducted. The records of 70 patients, treated with low-dose (2.5 mg/ml) 5-FU and 4 mg/ml TAC every 4 weeks (Group 1) or medium-dose (10 mg/ml) 5-FU and 4 mg/ml TAC every 4 weeks (Group 2), were analyzed. The Vancouver Scar Scale (VSS), vascularity, and thickness of hypertrophic scar at baseline and at 7th-treatment (each group received 6 treatment sessions) were compared. The ultrasound showed the large vascular distribution in scar margins. Both groups gained clinical improvement in VSS, vascularity, and thickness. Group 2 (medium-dose) exhibited significantly better improvement than Group 1 (low-dose). However, the overall side effects rate was 11.4% in Group 1, significantly lower than 31.4% in Group 2. Scar margins were suggested to be target sites for injection. Medium-dose (10 mg/ml) 5-FU + 4 mg/ml TAC could effectively reduce the thickness of hypertrophic scar; however, the side effects rate was also higher in medium-dose group than in low-dose group.

Comparison of Ultrasound-Assisted Low-Dose Versus Medium-Dose 5-Fluorouracil and Triamcinolone Acetonide in the Treatment of Hypertrophic Scar

<i>Background</i>. Acute generalized exanthematous pustulosis (AGEP) is a rare and serious adverse reaction of terbinafine. Understanding AGEP and terbinafine is primarily based on case reports. The purpose is to explore the clinical characteristics of terbinafine-induced AGEP, and to provide reference for clinical diagnosis and treatment. <i>Methods</i>. Case reports and original studies of terbinafine-induced AGEP were retrieved by searching Chinese and English databases from January, 1991, to May 31, 2022. <i>Results</i>. The median age of the 32 patients (17 males and 15 females) was 55 years (range: 6–84). The median time to onset of AGEP is 8 days (range: 1–77) and is usually accompanied by fever (>38°C) and elevated neutrophil levels (>8000/mm<sup>3</sup>). Four patients (12.5%) had oral mucosal involvement, and 10 patients (31.3%) developed postpustular desquamation. The lesions were mainly distributed in the trunk (43.8%), the whole body (34.4%), and the extremities (53.1%). Skin biopsy revealed subcorneal pustules (65.6%), intraepidermal cavernous pustules (43.8%), necrotic keratinocytes (15.6%), spongy hyperplasia (25.0%), neutrophil exocytosis (34.4%), and papillary dermal edema (40.6%). AGEP resolved completely in all patients at a median time of 12 days (range: 2–90) after discontinuation of terbinafine and symptomatic therapy. <i>Conclusion</i>. Clinicians should be aware that terbinafine-induced AGEP is a rare cutaneous adverse reaction. AGEP usually occurs within 2 weeks after administration and has a good prognosis after discontinuation.
