Relationships between the localization of the most effectively stimulated contact and the percentage of daily time spent in off states. All cases except one achieved the best improvement in the on-off phenomenon by bilateral deep brain stimulation of each paired combination of the subthalamic nucleus, zona incerta and field of Forel H.
