Vancomycin	15–20 mg/kg every 12 h
Linezolid	600 mg every 12 h
Daptomycin	6 mg/kg every 24 h
Teicoplanin	0.4–1.2 g qid (2 doses within the first 24 hours)
Ciprofloxacin	400 mg every 8 h or every 12 h
