(a) Spinal needle has pushed suture into bladder creating pulling loop. (b) Spinal needle passed through pulling loop via an adjacent puncture. (c) With needle through pulling loop, one free end of the suture is passed through spinal needle and thus the pulling loop. (d) Removal of spinal needle results in suture being snared by pulling loop. Subsequent retraction of pulling loop creates through-and-through suture which can then be tied fixing bladder to abdominal wall.
