(a) Grade III lesions, complete muscle fibre rupture, ruptured muscle with retraction, fascial injury, the bell-clapper sign, torn muscle fragments floating in the serohematic fluid. (b) Hematoma presenting as an anechoic circumscribed lesion; the total muscle diameter is 39 mm. (c) Decreased arterial diameter and blood flow, partial arterial disruption. (d) The result of this patient after surgery: complete muscle fibre rupture, fascial injury, several hematomas present, partial but continuous arterial disruption.
