Number of integrated care programs using at least one eHealth tool by country ()^a. ^aThe programs (on the whole and using at least one eHealth tool) were identified in the following 24 European countries: Spain, Greece, Iceland, Germany, Italy, Finland, The Netherlands, Denmark, Sweden, Luxembourg, Bulgaria, Cyprus, Belgium, Croatia, Malta, Lithuania, Norway, Ireland, England, Austria, Portugal, Slovenia, Latvia, and Switzerland. No eligible program was identified in Romania, Czech Republic, Hungary, Poland, Slovakia, and Estonia. Information on French programs was incomplete and thus excluded from the analysis.
