First, by definition, chronic disease is described by a sequence of events that (d)evolve an individual’s health state over a duration that is often far longer than any single visit to a healthcare facility.
Second, the sequence of these events do logically depend on each other as described by medical science.
Third, how any individual experiences this chronic condition is often entirely unique given their unique combination of social, behavioral, environmental, and biological risk factors.
Finally, this chronic condition often affects many aspects of an individual’s health that are often covered by disparate medical disciplines.
