Voriconazole 6 mg/kg IV q12 hr for first 24 hours, then 4 mg/kg IV q12 hr bridging therapy for allogeneic stem cell transplantation (clofarabine-cytarabine)
Voriconazole 4 mg/kg IV q12 hr + liposomal amphotericin B 5 mg/kg IV qday
Voriconazole 4 mg/kg IV q12 hr + liposomal amphotericin B 5 mg/kg IV qday
Died + 21 day of transplantation due to pneumosepsis
