Red blood cell alloantibodies  can cause severe delayed hemolysis, despite immunosuppression which they receive posttransplantation. A female patient with Hepatitis C-related chronic liver disease reported to our center for liver transplantation. During preoperative evaluation, she was found to have significant red blood cell alloantibodies which gave rise to problems during pre transfusion compatibility test. Stringent measures were taken by the transplant team to minimize blood loss during surgery. It was decided to have lower transfusion trigger for red cell transfusion, and blood conservation was done by intraoperative red cell salvage and use of antifibrinolytic agent. During immediate postoperative period, she developed drug-related immune hemolytic anemia. Presence of both warm autoantibodies and alloantibodies posed a big challenge for us to get cross-match compatible blood. She received 22 units of crossmatch compatible red cell transfusions during her hospital stay, which was uneventful. Hence, we reported this case.

Alloantibodies and Drug-Induced Immune Hemolytic Anemia in Liver Transplantation

Renal transplantation increases the probability of malignant tumors by about 2&#x02013;4-fold overall with a much higher rate for renal epithelial malignancy. Renal tumors in renal transplant recipients are commonly conventional clear cell or papillary renal cell carcinoma. Clear cell papillary renal cell carcinoma is a recently described unique renal epithelial neoplasm with scant eosinophilic or moderate amount of clear cytoplasm and pyknotic small nuclei oriented commonly toward the apical surfaces. No such tumor has been reported in renal transplant recipients. In this paper, we describe a clear cell papillary renal cell carcinoma involving bilateral native kidneys in a patient who had received a renal transplant 2&#x2009;years earlier. Clear cell papillary renal cell carcinoma commonly presents with low pathologic stage and lower Fuhrman grade and is clinically indolent. Additional cases are needed to evaluate the clinical behavior of this type of tumor in renal transplant recipients.

Parvovirus B19 is recognized as a rare cause of pure red cell aplasia (PRCA) in allogeneic stem cell (SCT) and solid organ transplant patients. We report a patient with Hodgkin&#39;s disease who developed PRCA due to parvovirus B19 after autologous SCT and who had an excellent response after treatment with gamma-globulin.

Pure Red Cell Aplasia due to B19 Parvovirus Infection after Autologous Stem Cell Transplantation

Cavernous transformation is a condition which has an acutely developing harmful effect over intestinal circulation compromising the patients' life before development of portal hypertension and its results. We present the case of phrenic artery hemorrhage after successful percutaneous portal vein stenting to treat cavernous transformation following LDLT. The patient survived the hazard complication with prompt surgery. Three factors may be related to the rare complication in the case were analyzed, including affluent new vessels around the diaphragm related to LDLT procedure, high puncture site allowing the diaphragm been injured, and anticoagulation given before the puncture and soon after the procedure. Cautions should be taken for the interventional procedures in this extreme condition. Phrenic Artery Hemorrhage after Percutaneous Portal Vein Stenting to Treat Cavernous Transformation Following Living Donor Liver Transplantation: A Case Report

The IMPACT-CABG study is the first Canadian randomized-controlled phase II clinical trial aiming to assess the effect of intramyocardial (IM) injections of CD133+-selected stem cells in patients referred for coronary artery bypass graft (CABG) with a &#8220;chronic&#8221; myocardial infarction and persistent left ventricular dysfunction. Patients are followed for 2 years with different imaging techniques including the stress magnetic resonance to evaluate the global and regional myocardial viability. Before the beginning of the randomization, the 5 first patients are treated in an open-label fashion to assess safety and feasibility of the IM CD133+ injections. Herein, we report the first Canadian patient treated with IM injection of CD133+ cells during CABG surgery as part of the IMPACT-CABG trial.
