DM: diabetes mellitus; LVEF: left vetricular ejection fraction; GFR: glomerular fraction rate. ACEi: angiotensin converting enzyme inhibitors. ARA II: angiotensin II receptor blockers. Mass, body mass index (BMI), muscle mass, fat mass and basal metabolic rate were obtained using bioimpedanciometry. HbA1c (glycated hemoglobin), is expressed in NGSP units (%) and in IFCC units (mmol/mol). PTH: parathyroid hormone. *Previous cardiovascular disease included myocardial infarction, coronary artery bypass graft surgery and percutaneous coronary revascularization. **Corrected for albumin. ***GFR was calculated with the MDRD equation.
