A CXR on a patient with COPD that had undergone a recent shoulder arthroplasty presented with acute shortness of breath. Diagnosis of left-sided pneumothorax was made clinically and from the CXR. A question of bullous emphysema was raised so an urgent CT scan was arranged. See Figure 12.
